Wellness Action Plan

This is your final project for this class and should be a plan for your life to keep you well and healthy both physically and mentally.  Each person’s plan will look different but must demonstrate a clear vision of what you want in your life and how you plan to get it.  This is due at the end of Module 5.  Include your name in the subject.

Use the template below. Start thinking of where you are chronologically. Think of all potential stressors at each age of life. Then put down as many strategies and activities you think might prove valuable to relieve that stress. Be cognizant of role restraints you might have at different times of your life. (i.e. Being a parent, taking care of parents as they age)
