You must complete an IRB application before beginning your DNP Project. You are asked to use IRBNet. There are several components of the IRBNet Applicaiton (Part A, Part B, and Attachments). You may complete Part A prior to Part B (actual project application). You may have several attachments. This will be dependent on your project focus. All students are required to submit the following attachments:

1.    NIH certificate (unless previously submitted)

2.    Application Part A

3.    DNP Application Part B

4.    Letter of Support from the location where the project will be implemented.

5.    Process Map

6.    Information sheet or what the participants will be told

7.    Instructor electronic signature

8.    HIPAA certificate (unless previously submitted)

I ONLY NEED ASSIST WITH APPLICATION PART B AND PROCESS MAP

You may also be required to complete an IRB application for your organization/agency. As your Faculty Advisor, I will guide you through this process.

Your Faculty Advisor MUST review your IRB Application prior to submission to IRBNet.

Here are some common IRB mistakes that you will want to avoid.  Student user profile missing “DNP Student” – without this identifier, the IRB is unable to quickly grant approvals

  • Missing advisor signature and being submitted by DNP student
  • Without letter of support
  • Without information sheet
  • Requests waiver of consent (does not apply to DNP Project

Submit email confirmation of successful IRB application as a pdf to this assignment.
